Housing application completed.

Supported with furniture from the available Personal Health Budget to promote her independence throughout chemotherapy.

Transport to financial appointment arranged through Communities 1st.

Registered with GP practice.

Arranged befriender, shopping support through Reach Out. Supported with picking up prescription glasses when client was too weak.

Liaising closely with Macmillan benefit advisor to make sure client has the right support as had identified a slight language barrier and involved professionals who started to work closely were made aware so the right support could be put in place
