Wellbeing Checks After Discharge
We contact all patients aged 65+ who leave hospital without formal care. This service helps to:

Build confidence after discharge

Waiting Well


We support people waiting for surgery by helping them stay healthy, informed, and prepared.
We help with:

Weight management

Smoking cessation

Reducing alcohol intake

Managing pain and mobility

Reducing anxiety and uncertainty
This proactive approach reduces cancellations and improves surgical outcomes.

Frailty Support
Working closely with Princess Alexandra Hospital, we contact frail patients shortly after discharge to identify emerging needs early.
Support includes:

GP liaison

Emotional support
This service has already prevented avoidable readmissions and improved patient safety.

Age UK Hertfordshire

At the start of WWII, the National Council for Social Services set up an Old People’s Welfare Committee to support older people evacuated from London and other cities. The Hertfordshire Old People’s Welfare Council took on the work and continued after the war, registering as a charity in 1966 and then becoming Age Concern Hertfordshire and then Age UK Hertfordshire.
British Red Cross

The British Red Cross was founded in 1870 and provided humanitarian services during the Franco-Prussian War. During the World Wars, the Red Cross was active in Hertfordshire providing nurses, hospitals, courses and supporting refugees, evacuees, the wounded and homeless. The British Red Cross operates under a Royal Charter granted in 1997.

Carers in Hertfordshire was formed in 1995 by local carers, and was registered as a charity in 2001.
Dacortium

Dacortium was founded in 2013 and is a partnership of charities active in Dacorum: Relate, CAB, Mediation Hertfordshire, DrugLink, DENS, the Centre in the Park, and Community Action Dacorum.
Watford & Three Rivers Trust

W3RT was founded in 1974 as Watford CVS and was registered as a charity in 2001.
